MauniGGZ is a limited liability company incorporated under the laws of The Netherlands. What personal data we collect and process? 

This Privacy Statement provides information about collecting, storing and processing of your personal data that you disclose to us when you use our services. The type of personal information we collect may depend on how you interact with us and our platform and which services you choose to use. Generally, we collect, store and use the following categories of personal information: Your basic information 

When you sign up with MauniGGZ, a staff member will ask if a medical file may be opened for you. In this file, the following information will be recorded: basic information name, address, and postal code, Citizen Service Number (BSN), Date of birth, Phone number, Email address, Health insurance company where you have your basic insurance. 

o Data about your health: Examples of medical data include diagnosis, medication use, and medical history. 

Examples of health data include: substance use, smoking habits, sexual life (secondary diagnosis), criminal history (risk assessment), living arrangement, lifestyle, education level, country of origin, etc. 

o The Citizen Service Number (BSN) : BSN is requested during registration to verify whether you have valid basic health insurance. To prevent identity confusion, MauniGGZ is legally obligated to use your BSN in communication with other healthcare institutions, such as the general practitioner, and with entities like insurers, laboratories, and pharmacies. 

o Your use of the MauniGGZ website: If you use the MauniGGZ website, MauniGGZ will store data about you. For example, your IP address, information about your visit to the website, and the device used to visit the website. How is your data collected? MauniGGZ mainly receives data from you. No data is collected from public sources. If you are a client, it is possible that MauniGGZ may receive data about you from one of the parties listed below. 

o A Referrer: A referrer, usually your general practitioner (GP), can refer you MauniGGZ. The referrer provides a referral that includes information legally required. The referral can be provided to you on paper or digitally to MauniGGZ via secured emailing system. Before the referrer registers you, they need your consent. You can also receive a referral from an occupational physician, a hospital doctor, or another mental health care institution. 

o Parties MauniGGZ collaborates with: MauniGGZ collaborates with other parties, with your consent, it is possible that MauniGGZ receives and shares data with these healthcare professionals. 

What is the purpose of processing your personal data? 

We process your personal data in accordance with the provisions of Data Protection Regulation. We will only use and process your personal data we obtain in the context of contracted care and support services. We intend to use and process your personal data under the following conditions: 

• For the performance of contractual obligations. 

• Where we are required to comply with legal or regulatory requirements. 

• When you have provided us with your explicit and informed consent to do so. 

In case we rely on consent as a legal basis for processing your personal data, please note that you have the right to withdraw your consent at any time by contacting at admin@mauni-ggz.nl. However, please consider that withdrawing the consent may affect the legal bases of processing of your data, hence we may not be able to provide certain services to you. If this is the case, you will be advised accordingly at the time you withdraw your consent. 

What Are the Security Measures?

We are committed to maintain organizational and technical security measures to protect the personal data we hold about you. We have implemented a set of controls and relevant policies and procedures to prevent the personal data we hold about your from being accidentally or deliberately compromised. 

How Long do we Retain Your Personal Data? 

Your personal data is stored in accordance with the provisions of the contractual agreements and/or applicable statutory obligations. We maintain adequate retention policies and procedures, so that your personal data are deleted after a reasonable and prescribed time. The data will not be stored longer than is strictly necessary to achieve the purposes for which the data have been collected. We adhere to legal retention periods. When there is no legal retention period, Mauni GGZ does not keep data longer than necessary for performing the task. 

o MauniGGZ is obliged to retain medical records for 20 years from the day of the last change in the file (or longer if there is an ongoing claim or lawsuit), or for a shorter period if the Data Subject has invoked destruction. 

o Mauni GGZ is required to retain financial data for 7 years from the end of the calendar year. 

o Non-medical data is kept for a maximum of 2 years after the client’s departure. 

o Emails are kept for a maximum of 6 months after receipt unless there is a need to keep Sharing of Personal Data with Third Parties. We do not transfer your personal data to third parties unless it is mandatory by law or a court order, in order to protect the rights of the parties involved in the performance of the agreement or if you give your consent to do so. 

o Cookies are files with a small amount of data, which may include an anonymous unique identifier. We use functional cookies that are essential for technical operations necessary for the users’ navigation on the platform. These cookies also support the website security and basic functionality. You can set your browser in order not to accept cookies. However, in certain cases, some of our website features may not function as expected. What are your Data Protection Rights? 

You have the right (under the certain circumstances and subject to the exceptions), pursuant to Data Protection Act to: 

• The right to access: You have the right to access and request copies of your personal data. 

• The right to rectification: You have the right to request us to ratify any incomplete or inaccurate data we hold about you. We reserve the right to verify the accuracy of the new data you provide to us. 

• The right to erasure: You have the right to request us to erase your personal data. 

• The right to restrict processing: You have the right to request us to restrict the processing of your personal data. 

• The right to object to processing: You have the right to object to the processing of your personal data.
